Provide one of the following as proof of residency/ home address:
- Postmarked envelope, postcard, or magazine label with name and date
- Driver's license issued within past 6 months
- Utility bill (gas, electric, cable), bank statement, or letter from a government agency which contains your name and home address (not a P.O. Box)
- Letter/lease/rent receipt with home address from landlord
- Property tax records or mortgage statement
The address on this document should match the home address you provide on the application.
